You need the game to be patched to enable the LAA flag to use more than the default of 2GB allowed to 32-bit games. If you are having a crash after applying the 4GB patcher to the game, more than likely you have obtained it from GOG instead of Steam.
Well the Steam envelope for the FalloutNV.exe does not tolerate any tampering and will error out if you modify it that way, so this tool was created by MonochromeWench to get around that limitation. It is recommended to copy the FNV4GB.exe and fnv4gb_helper.dll to the New Vegas installation directory. Doing so will allow the tool to work with ENB, SweetFX, FXAA injector and other such utilities with a minimum of hassle. However it is not necessary (YMMV).

Therefore, the problem is with Steam, since a cracked version and thus Steam-independent has no trouble. There is also a supposed fix which involves changing the name of FNV4GB and launching Fake Fullscreen: i'm not sure, but what that fix does is just disallowing FNV4GB to run, since if you start it directly, still get the same errors. ALso, deleting FNV4GB fixes the issue (though the game is way more unstable) So, by now, only thing I guess works properly is cracking the game. If I can find one and test it, I'll post here if it works. Hope it helps somebody. EDIT: Cracking the retail copy from Steam DOES work if you set FNV4GB to compatibility mode for Win7.
