If I build with full optimizations and without generating debug info, which is more or less what Release mode does, but create the ID3D11Device with D3D11_CREATE_DEVICE_DEBUG flag set, the break still occurs. If I don't use the DirectX debug layer, the program terminates normally. I don't think it solves the problem, though - it probably just doesn't get reported.
